Community Connections, Inc. (“CCI,” “we,” “us,” or “our”) respects your privacy. This Privacy Notice (“Notice”) describes how we process Personal Information (defined below) that is provided, collected, or disclosed through the websites, online platforms, forms, accounts, and consumer-facing portal features that link to or reference this Notice (collectively, the “Site”).
The Site allows individual patients, consumers, clients, or their authorized representatives to submit information for potential inclusion in the Southwest Colorado Cares (“SWCC”) Community Care Hub and to support referrals, service coordination, and connections with medical, behavioral health, housing, food, benefits, and other community resources.
Please read this Notice carefully to understand our policies and practices regarding your Personal Information and how we will treat it.
Important HIPAA Notice
This Notice does not apply to Protected Health Information that is subject to HIPAA. To the extent information you submit or information maintained through SWCC is Protected Health Information subject to HIPAA, including information maintained by a HIPAA Covered Entity Participant or by CCI as a Business Associate, that information is governed by HIPAA, the applicable Covered Entity’s Notice of Privacy Practices, any applicable Authorization or Consent, and applicable agreements, and not by this Notice. This Notice applies only to Personal Information that is not subject to HIPAA.
Personal Information We Collect
We collect several categories of Personal Information from and about users of our Site. “Personal Information” means information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked with an identified or identifiable person and that is protected by Applicable Law. The categories of Personal Information we collect may include:
- Contact and Account Information – If you submit an inquiry, register for an account, request services, or provide information on or through our Site, we may collect your contact information, including your name, mailing address, email address, phone number, login information, and other account information.
- Identity and Demographic Information – We may collect information such as date of birth, age, language preference, household information, accessibility needs, or other information you choose to provide to help identify you, support referrals, or connect you with appropriate resources.
- Service, Referral, and Community Resource Information – We may collect information about the services, referrals, resources, programs, eligibility criteria, needs, preferences, requests, communications, and outcomes you submit or ask us to help coordinate through the Site or SWCC.
- Sensitive Information You Choose to Provide – Depending on what you choose to submit, the Site may collect information about health conditions, behavioral health, disability, housing, food access, benefits, financial circumstances, citizenship or immigration-related information, family or household needs, student information, or other sensitive information. We process Sensitive Data only with your consent or as otherwise permitted by Applicable Law.
- Documents and Uploaded Content – If the Site permits uploads or free-text submissions, we may collect information contained in documents, notes, forms, files, or other content that you choose to provide.
- Usage Information – When you use our Site, we may automatically record information, including your Internet Protocol address, device and browser information, referring URLs, pages viewed, time and date of access, and other information regarding your interaction with our Site.
- Communication Information – We may collect Personal Information contained within your communications with us via the Site, chat functionality, email, telephone, text message, chat, or otherwise, and in certain cases we may use third-party service providers to help us manage those communications.
How We Collect Personal Information
- Directly From You – We collect Personal Information that you provide to us directly, for example, if you create an account, submit information for inclusion in SWCC, request a referral or resource connection, contact us, complete forms, or otherwise use the Site.
- From Authorized Representatives and Participants – We may collect Personal Information from your parent, guardian, caregiver, personal representative, or other person you authorize, and from SWCC Participants or community partners involved in referrals, service coordination, or resource connections.
- From Service Providers and Through Online Technologies – We may collect Personal Information through service providers that support the Site and through cookies and similar technologies used to operate, secure, maintain, and understand the use of the Site.
How We Use Personal Information
To the extent permitted by Applicable Law, we use Personal Information:
- To provide, operate, maintain, secure, and administer the Site and SWCC-related features, including creating and maintaining accounts, verifying information, supporting referrals, coordinating service requests, and helping connect individuals with Participants and community resources.
- To receive, evaluate, route, and respond to information, inquiries, forms, referrals, requests for assistance, and other communications that you submit through the Site.
- To communicate with you, including responding to your questions, providing administrative or transactional messages, and sending information about your account, referrals, requests, or Site updates.
- To maintain or improve the Site, troubleshoot issues, understand how users interact with the Site, test features, maintain records, and support quality, security, and compliance activities.
- To create aggregated or de-identified information, where permitted by Applicable Law, for reporting, planning, evaluation, quality improvement, and similar purposes.
- For legal, security, or safety reasons, such as protecting our and our users’ safety, property, or rights; complying with legal requirements; enforcing our terms, conditions, and policies; detecting, preventing, and responding to security incidents; and protecting against malicious, deceptive, fraudulent, or illegal activity.
- To fulfill any other purpose for which you provide the information, including purposes described when you provide the information or give your consent.
How We Disclose Personal Information
We may disclose aggregated or de-identified information about our users, meaning information that does not identify any individual, without restriction. We may disclose your Personal Information with your consent or in the following circumstances:
- CCI Personnel – We may disclose Personal Information to our employees, contractors, and other personnel who have a need to know the information for the purposes described in this Notice.
- Participants and Community Resource Partners – We may disclose Personal Information to SWCC Participants, providers, agencies, organizations, or other community resource partners as directed by you, with your consent, as reasonably necessary to process your referral or request, or as otherwise permitted by Applicable Law.
- Service Providers – We disclose Personal Information to service providers that support the Site and SWCC-related features, including Database Vendors, Hosting Providers, technology providers, analytics providers, communications providers, security providers, and other vendors that process information on our behalf.
- Legal Obligation or Safety Reasons – We may disclose Personal Information when we have a good faith belief that disclosure is reasonably necessary to satisfy or comply with law, regulation, legal process, or enforceable governmental request; enforce or investigate a potential violation of our terms or policies; detect, prevent, or respond to fraud, security, or technical concerns; support auditing and compliance functions; or protect the rights, property, or safety of CCI, Participants, users, or the public.
- Corporate Transaction – We may disclose Personal Information as part of a corporate transaction, such as in connection with the sale of part or all of our assets or business, the acquisition of part or all of another business or another business’ assets, or another corporate transaction, including bankruptcy.
- Other Disclosures With Your Direction or Consent – We may disclose Personal Information when requested by or consented to by you, or for the purposes described when you provided the information.
We do not sell Personal Information or process Personal Information for targeted advertising through the Site.
Cookies and Other Tracking Technologies
We and our service providers may use cookies and similar technologies to collect usage and browser information about how you use our Site. These technologies may help us operate and secure the Site, verify system and server integrity, remember preferences, generate statistics, understand Site traffic and usage, troubleshoot problems, and improve Site functionality. We do not use these technologies to sell Personal Information or to deliver targeted advertising.
You can change your cookie preferences by customizing your browser or device settings to refuse all or some cookies or to alert you when websites set or access cookies. If you disable certain cookies, some parts of our Site may not function properly. Where required by Applicable Law, we will recognize valid opt-out preference signals, such as Global Privacy Control, for processing activities to which such signals apply.
How Long We Keep Your Personal Information
We retain Personal Information for as long as needed to provide the Site and SWCC-related features, fulfill the purposes described in this Notice, comply with our legal obligations, resolve disputes, enforce agreements, maintain security, and support operational, audit, and recordkeeping requirements. Criteria we use to determine retention periods include the nature of the information, the purposes for which it was collected, our relationship with you, applicable legal requirements, and whether we have an ongoing need to retain the information.
Links to Third-Party Websites
Our Site may link to third-party websites, resources, or services. We are not responsible for the practices employed by websites or services linked to or from our Site, including the information or content contained within them. Any Personal Information you disclose to a third party will be handled in accordance with that third party’s privacy policy.
How We Protect Personal Information
We have implemented commercially reasonable measures designed to secure Personal Information from accidental loss and from unauthorized access, use, alteration, and disclosure. Unfortunately, transmission of information via the internet is not completely secure. Despite these efforts, we cannot guarantee the security of Personal Information during transmission or storage.
Exercising Your Privacy Rights
Depending on where you live, you may have the following rights with respect to your Personal Information under Applicable Law, including, where applicable the Colorado Privacy Act:
- Access – The right to confirm whether we process your Personal Information and to access such Personal Information.
- Portability – The right to obtain a copy of Personal Information that you previously provided to us in a portable and, to the extent technically feasible, readily usable format.
- Correction – The right to request that we correct inaccuracies in your Personal Information, taking into account the nature of the information and the purposes of processing.
- Deletion – The right to request deletion of Personal Information that we have collected about you, subject to certain exceptions.
- Opt Out of Certain Processing – The right to opt out of the processing of Personal Information for targeted advertising, sale of Personal Information, or profiling in furtherance of decisions that produce legal or similarly significant effects. We do not sell Personal Information, process Personal Information for targeted advertising, or profile individuals in furtherance of decisions that produce legal or similarly significant effects through the Site.
- Appeal – If we deny your privacy rights request and Applicable Law gives you the right to appeal, you may appeal our decision using the instructions we provide in our response.
To exercise your privacy rights, please contact us at connect@swcoloradocares.org. We may need to verify your identity before fulfilling certain requests. If we cannot verify your identity, we may request additional information. If you use an authorized agent, we may require proof of the agent’s authority and may also require you to verify your identity directly with us, as permitted by Applicable Law.
You will not be discriminated against for exercising the rights listed in this Notice. However, if you ask us to delete information, withdraw consent, object to processing, or choose not to provide certain Personal Information, we may be unable to provide some or all Site features, referrals, resource coordination, or related services to you.
We endeavor to respond to privacy rights requests within the time required by Applicable Law. We may deny certain requests, or fulfill some requests only in part, as permitted or required by law.
Children’s Privacy
The Site is not intended for use by children without appropriate involvement of a parent, legal guardian, or other person authorized by Applicable Law. We do not knowingly collect Personal Information from children in violation of Applicable Law. If you believe we may have collected Personal Information from a child without required consent or authorization, please contact us at connect@swcoloradocares.org.
Changes to this Notice
We may modify or update this Notice from time to time, so please review it periodically. If we make material changes to how we treat Personal Information, we will notify you according to Applicable Law. Unless otherwise indicated, any changes to this Notice will apply when posted to our Site.
