Southwest Colorado Cares (SWCC) is bringing together community organizations, service providers, and government agencies to address the social drivers of health. As a Care Partner, your organization becomes part of a collaborative network to streamline referrals, strengthen care coordination, and help residents find and receive the services they need.
Whether you support housing, food, transportation, aging and disability services, healthcare, or other community needs, there’s a place for you in the SWCC Care Partner Network. SWCC serves people throughout Archuleta, Dolores, La Plata, Montezuma, and San Juan Counties.

Choose Your Path to Partnership
Care partners may choose to participate at one or more of these levels:
1. Regional Resource Directory: Include information about your organization and services in this comprehensive guide for use by other care partners and residents. SWCC is collaborating with 211 Colorado, so you only have to update your information once and it will be reflected in both directories. Participation in the directory is a prerequisite for participating at all other levels.
2. Managed Referrals: Use a webform to refer residents to the SWCC care navigator and receive referrals from the care navigator by phone, fax, or email. Notify the care navigator of referral outcomes to “close the loop” and make sure people get the services they need.
3. Electronic Referrals: Use the SWCC technology platform to securely send e-referrals to and receive e-referrals from the SWCC care navigator and other care partners. View your cases and record referral outcomes to track cases to completion.
4. Care Coordination: Use the SWCC technology platform to its full potential for managing your caseload and coordinating service delivery for people served by your organization and other care partners. We can also explore the potential for integration with electronic health records.
What are the Expectations of Care Partners?
All care partners agree to observe the shared values and core commitments outlined in the SWCC Community Charter, as appropriate to the organization’s level of participation.
Shared values include equity, accountability, collaboration, and person-centered practice. Core commitments include maintaining accurate resource information, engaging professionally with partners and people, and fostering a strong community.
