Southwest Colorado Cares program componentsSouthwest Colorado Cares (SWCC) is a regional Community Care Hub that makes it easier for residents to access resources to meet basic needs such as food assistance, housing support, transportation, financial aid, aging and disability services, healthcare, and more.

The SWCC Community Care Hub features an online resource directory, care coordination and navigation services, community ambassadors, and a care partner network to create one coordinated system so that Southwest Colorado residents can stay healthy, stable, and thriving.

SWCC is freely available to everyone in Archuleta, Dolores, La Plata, Montezuma, and San Juan counties, with extra support for people with disabilities and people insured by Medicaid.

We are a program of Community Connections, Inc., a nonprofit serving southwest Colorado for 40 years. SWCC is funded by a grant from the Colorado Office of eHealth Innovation, and other generous donors, as a pilot site for the Colorado Social Health Information Exchange (CoSHIE).
